Accutane / Isotretinoin recovery — week by week
What healing actually looks like — week by week — for accutane / isotretinoin.
Recovery timeline
- Week 1Initial 'purge' — acne typically worsens before improving. Dry lips immediately.
- Month 1Severe dryness peaks (lips, nose, hands). Lipids and liver enzymes checked monthly.
- Months 2–4Acne dramatically improves. Skin sensitivity peaks. No waxing, lasers, or aggressive exfoliation.
- Months 4–6Course complete for most. Cumulative dose target ~120–150 mg/kg.
- Months 6–12 post-treatmentLong-term clearance for ~70% of patients. Some need a second course.
Quick reference
- Back to work
- Not surgical — no time off. Physical activity unaffected.
What's normal vs. what to call about
Some swelling, bruising, asymmetry, numbness, and tightness are expected in the first weeks. Sudden severe pain, fever above 101°F, drainage, spreading redness, or one-sided swelling that worsens after improving warrant a same-day call to your surgeon.
